PATIENT SERVICES REPRESENTATIVE COORDINATOR
UBMD Primary Care is seeking full time Patient Services Representative Coordinator (PSRC) for our Sheridan outpatient clinic located at 2465 Sheridan Drive, Tonawanda, NY 14150. The PSRC is responsible for the management of the front desk. Responsibilities include managing day-to-day PSR employee needs, workloads, and schedules as well as greeting and registering patients, answering phones, scheduling appointments, entering patient charges and demographics, retrieving and updating medical records, collecting fees and performing general administrative and other duties as assigned by the Operations Manager. Will order office supplies and manage organizational needs for best efficiency, as well as manage office banking responsibilities. Will manage job assignments for PSR's to monitor workloads (prior authorizations, referrals, new patient process, scanning) and adjust as needed. Provide mentorship and training to PSR's. Assists in quality metric projects such as daily no-show, error weekly work, procedure book management, alignment forms, etc.). Will work as a PSR while managing coordinator responsibilities with duties such as scheduling patient appointments, capturing and verifying patient demographics, insurance, etc., greeting and directing patients, etc.
High school diploma or GED required, Associate's degree preferred. One to two years' experience working in medical reception, preferably in an outpatient clinic setting. Medical terminology preferred and supervisor experience required. Excellent communication, organizational and multi-tasking skills required.
Schedule: Monday through Friday, 8:15am - 4:15pm with a rotating evening shift (along with other staff).

NURSE PRACTITIONER/PHYSICIAN ASSISTANT (APP)
UBMD Primary Care seeking full-time Nurse Practitioner or Physician Assistant (APP) to float between our two outpatient clinics in Tonawanda and Williamsville (will not work at both sites on the same day). The APP will provide primary and episodic care to patients under the supervision of a physician. Responsible for delivery of quality care to patients, while emphasizing health promotion and/or disease prevention through patient education and coordination of care. Will interpret diagnostic test results, compile and record patient medical data, make diagnoses and decisions about management and treatment of patients. The APP prescribes therapy or medication with physician approval, as well as administer or order diagnostic tests such as x-rays, electrocardiograms and lab tests. Graduate of accredited PA or NP program. Current NYS license as PA or NP certified by the NYS Education Department. Minimum of 2 years' experience in health care delivery, preferably in an outpatient clinic setting and preferably in family medicine or primary care. LICENSED PRACTICAL NURSE (LPN)
UBMD Primary Care seeking one (1) Licensed Practical Nurse (LPN) for busy outpatient clinic in downtown Buffalo at the Conventus Medical Office Building. Qualified candidates should have a minimum of one (1) to two (2) years experience in nursing, preferably in an outpatient clinic setting. Experience with Electronic Medical Records (EMR) experience and triage/phone experience helpful. Candidates should also have excellent communication and multi-tasking skills required.

MEDICAL OFFICE ASSISTANT
UBMD Primary Care seeking full-time Medical Assistant (MA) to perform clinical duties at our outpatient clinic in Williamsville. Responsible for ensuring timely and efficient rooming of patients, performing clinical intake on patients by checking vitals, preparing patients for exams, procedures and/or treatments, as well as obtaining and evaluating patient's history in EMR system. Will assist with provider orders and call backs under the direction of a nurse, APP or physician. Certified Medical Assistants (CMA or CCMA) or degree preferred. May substitute a minimum of 2 years' experience in lieu of certificate/degree. Experience in an outpatient clinic setting preferred. Excellent communication, organizational and multi-tasking skills required. EMR experience required.
Schedule will be 8:30am - 4:30pm Monday through Friday with a minimum of one evening (Monday) per month in rotation with the other clinical staff. Office located on the corner of Hopkins & Klein Roads in Williamsville. Pay range is $18 to $19.50/hour depending on experience. LPN QUALITY SPECIALIST
UBMD Primary Care seeking part time LPN Quality Specialist (2 days per week) to work with care team members on meeting and maintaining quality measures set by payers and various regulatory programs. Will facilitate practice redesign efforts and support improved patient outcomes, as well as assist with increasing patient, provider and health care team satisfaction and efficiency within the practice. Will perform outreach to patients who have been recently discharged from the hospital or emergency room to reconcile medications, connect patient with appropriate follow up and answer questions about transition of care process. Conducts previsit planning on upcoming appointments to ensure necessary information is gathered for providers.
Must have a current and valid NYS LPN license. Minimum of 2 to 3 years of experience working in an outpatient clinic or primary care practice preferred. Experience working on quality measures a plus! Must have excellent communication and multi-tasking skills. Knowledge of PCMH and patient education preferred.
Position can be remote after training period with availability to be on site for meetings as needed. Schedule will be 7.5 hours on Tuesdays and Fridays OR Thursdays and Fridays. RN TRIAGE
UBMD Primary Care seeking full-time Registered Nurse for triage at its busy outpatient clinic at the Conventus Medical Office Building downtown. The RN will manage and respond to patient calls seeking medical information, medical attention and/or matters of an urgent nature. Will be responsible for assisting patients via phone interaction and/or on site by assessing needs, offering clinical options, referrals to health care providers, services and community resources. Provides telephone triage interactions that may require assessment, patient education and crisis intervention, focusing on the assessment and disposition of symptom-based calls. Will assist with medical refill management and forms as well as assisting team of medical assistants and LPN’s with clinical questions, as needed. Will be responsible for assisting in the weekly allergy clinic.
Ideal candidate will have an active NYS RN license with a minimum of 3 to 4 years’ experience as an RN, preferably in an outpatient clinic setting. Experience in triage required. Excellent communication, multi-tasking and organizational skills required. Must have a high attention to detail.

UBMD Primary Care physicians are responsible for maintaining an active clinical practice at an ambulatory clinic and/or designated hospital. Responsibilities include: Prescribe or administer treatment, therapy, medication, vaccination and other specialized medical care to treat or prevent illness, disease or injury; order, perform and interpret tests and analyze records, reports and examination information to diagnose patients’ condition(s); coordinate timely follow up of patient requests for services regarding prescription requests, referrals, diagnostic testing and appointments for sick visits; and supervise nurse practitioner(s) and/or physician assistant(s), when applicable and required. Must have active NYS medical license, valid DEA registration, board certification or eligibility in Family Medicine, completion of graduate medical education program and completion of residency training program.
